Most practices calculate in-house cleaning cost as hourly wage times hours worked. That number misses a significant share of the real expense — and the gap is larger in a medical facility than in a standard office, because of the added training and compliance requirements.
Costs Practices Often Overlook
- Payroll taxes and benefits: employer-side taxes, unemployment insurance, and any benefits add meaningfully on top of the base wage.
- Bloodborne pathogen training: staff who clean clinical spaces need OSHA-required training on bloodborne pathogen exposure — a cost and a liability that doesn't apply to general office cleaning.
- Turnover and retraining: cleaning staff turnover is high industry-wide, and every departure in a medical setting means retraining someone on infection-control protocols specific to your facility.
- Supplies and equipment: EPA List N-registered disinfectants and the equipment to apply them correctly cost more than general-purpose cleaning products.
- Supervision time: someone on staff has to manage scheduling, verify protocol compliance, and handle callouts — real management overhead that's easy to undercount.
- Liability exposure: an in-house cleaning employee injured on the job affects your workers' comp rates directly. An outside provider's insurance covers their own staff.
- Inconsistent coverage: when your in-house cleaner is out sick, exam rooms don't get disinfected on schedule. A commercial provider carries backup staff for exactly this reason.
When In-House Can Make Sense
For very large multi-location health systems with dedicated facilities staff, an in-house team can work.
